The physical/medical history form can be found under the "Download and Print" tab. The athletic handbook is now included as part of the online forms. Physical/medical history form must be downloaded, printed, completed and turned in, along with the and catastrophic insurance fee, to the trainers office in person by the first day of school.
Paperwork or money should NOT be dropped off in the front office, sent through mail, or email. It must be submitted in person.
All student-athletes who see a physician (including ER visits) for ANY reason must bring a note from the physician to the GHS trainers that states any restrictions the athlete may have as well as any activity that is allowed/encouraged.
Any student-athlete who has been evaluated by a physician will not be allowed to participate in any capacity (game, practice, etc.) until a note from the physician has been provided to the GHS Athletic Trainer.
We encourage all athletes to take the GHS Physician's Note to the doctor's appointment, this form is tailored towards athletics and also dictates which activities, treatments, and therapy the doctor may encourage or discourage.
All student-athletes suspected of having sustained a concussion may receive academic accommodations from their treating physician.It is recommended that these be updated every 7 days, and at a maximum of every 14 days. All documentation for academic accommodations will be forwarded to the school nurse for distribution to the student-athlete's teachers.
After the evaluation of a suspected concussion by the Staff Athletic Trainer or physician, please use the following form to monitor your child's symptoms daily.
This form must be submitted, at least 24 hours in advance, to Coach Ginger in the event an athlete needs to ride home from a meet with their parents. Under no circumstances will an athlete be released to ride with anyone other than their parents, or a person designated by their family.
